Mostly okay, things on top shelves are a nuisance, even if you can stand up the combination isn't designed to allow it (you end up with the edge of the trolley jammed into your stomach if you do try). The clown castors on my chair (7 inches - who the Hell puts 7 inch castors on a manual chair?) are so big they catch on the frame of the trolley when they try to swivel right around, occasionally forcing me to have to physically push the trolley sideways to let the castor get around, but that's a design flaw on my chair, rather than the cart. And if I buy too much I may end up with my forward vision obscured once it's bagged and back in the cart - I've rolled straight into a three feet high bollard because I couldn't see it.
The other irritation is social, checkout staff who don't believe me when I say I'm okay to pack stuff myself. The most irritating incident was one who dived into the back bag on my chair when I was reaching into it for more carrier bags - would she have done that with a non-disabled customer's handbag?
But it means I can shop for myself without getting back to the house and collapsing into exhausted sleep with the shopping still unpacked, which was the situation that forced me to give up shopping for myself, so overall it's a major plus!
